OFF-SITE LOCATION AND TRIP REPORTING FORM

For annual Clery reporting purposes, the ºÚÁϳԹÏÍø must collect crime statistics for those areas the university owns or has control over.  Department representatives must complete this form for any university-sponsored trips, programs or activities conducted at an off-campus site that include students, regardless of whether an incident occurred at the site.  Student teaching, medical practicums and College Credit Plus (CCP) programs do NOT need to be reported unless an agreement exists between ºÚÁϳԹÏÍø and the site for a specific space at the site to be controlled by ºÚÁϳԹÏÍø.
