Patient Portal Help
Introduction
There are four types of forms that you may be asked to complete when visiting DeWeese Health Center. The email you receive when scheduling your appointment(s) will indicate what forms you need to complete prior to coming to your appointment.
The types of forms you may be asked to complete are listed below.
Patient Portal Electronic Form
These are our most common type of form. These forms are web-based forms housed in the DeWeese Health Center Patient Portal. These forms are the preferred method for you to complete as they are generally simple, straight-forward, and can be electronically signed by you typing your name. These forms are found in the DeWeese Health Center Patient Portal under the "Forms" tab once you sign in.
